Abstract

43 year old male patient history of undergoing RK in both eyes .He was no PL in left eye with old retinal detachment. BCVA 6/60 in right eye Nuclear sclerosis grade 3 . Retinal examination revealed old barrage laser which needed further augmentation anteriorly.

After thorough augmentation of barrage he was posted for cataract surgery.

Routine phaco was started .While refiling the anterior chamber with viscoelastic the cannula inadvertently dislodged into the AC.

Presuming a posterior capsular break, phaco was done by prolapsing the nucleus out of the bag. PC rent was identified at the end of the cortical wash.

The RK incision at the entry wound started giving away.

Without any delay ac was reformed and a 3 piece IOL was placed in the sulcus. No visible vitreous was found.The RK incision was sutured.

2 week follow up showed vit prolapse in AC. He was posted for anterior vitrectomy .Post surgery patient had BCVA of 6/9 with a ON retina.
