Abstract

A 55 year old male came with vision of finger counting close to face in the left eye for 1 year, after undergoing cataract surgery 4 years ago. The anterior capsule was fibrotic, contracted. The PCIOL was in the bag, opacified and milky white. Under informed consent, a 6 mm scleral incision was given 3 mm from limbus at 12 o’clock. A corneoscleral tunnel was made and AC entered. Viscoelastic was injected. The PCIOL was moved, the haptics and capsular bag were adherent. The haptic of the IOL were cut and the optic was removed. A hydrophobic, aspheric, foldable PCIOL was placed on the capsular bag. The vision of the left eye was 6/12 with -1.50 cyl at 90 degrees, stable after 6 months.

In such cases of opacified PCIOLs, we have removed the PCIOL and exchanged it. In this case, it would have been unwise to attempt that with a fibrotic and adherent bag-haptic combination. In such cases, it is best to leave the haptic and implant the new IOL over the bag through a corneoscleral tunnel.
