Seemingly innocuous epibulbar masses might just be alive and kicking! Ocular cysticercosis is a very common entity in India. It may present as a subconjunctival, intraocular, ocular adnexal, optic nerve, or eyelid mass. Its varied presentation can lead to misdiagnosis. The various stages of evolution and later degeneration of the cyst may appear similar to episcleritis, nodular scleritis, dermoids, lipodermoids, epibulbar choristoma, or lacrimal gland lesion in the epibulbar area. This video demonstrates a misdiagnosed, perplexing presentation of ocular cysticercosis, as an epibulbar hard mass, investigations done to finally arrive at a diagnosis, its treatment, and eventual excellent outcome. The treatment, in this case, was a simple, controlled evacuation, emphasizing on ‘Less is more’. The varied forms or presentation of this parasite, its diagnostic clinchers and optimal treatment modalities depending on the location of the lesions are also demonstrated
