Forty three year old, male patient with unstable PDR in both eyes underwent a 3 port 25G sutureless transpars plana vitrectomy in right eye. Fibrovascular proliferation causing TRD was present nasally. A 25G spatula was used to dissect the membrane entirely which was then trimmed with a vitrectomy cutter. Neovascular fronds were cauterised with endocautery. PVD induction was confirmed by staining with preservative free Triamcinolone acetonide and vitrectomy was completed. ILM peeling centered on the foveal was done after staining with brilliant blue dye. Endolaser was performed and Silicone oil was injected. In this video we demostrate that in select cases adequate membrane dissection can be successfully achieved with the use of a 25G Spatula alone, without the need for scissors or other bimanual techniques.
