Abstract

A 19yr male presented with pain, decrease of vision in RE following blunt trauma with a ball 2 days back. On examination, vision RE FCCF, microcystic edema, hyphaema, traumatic mydriasis with IOP 52 mmHg; LE 6/6, IOP 14mmHg.RE AC wash was done and conservatively managed. His RE vision improved to 6/18 p; fundus CDR 0.7 but IOP was 37 mmHg. As his IOP was not controlled with maximal medical therapy for >1 month, he underwent trabeculectomy with Mitomycin C and releasable suture in RE. Patient lost to follow up. On PO 3 weeks, RE moderate AC, large bleb, choroidal’s with hypotony maculopathy. IOP was 2mmHg and Seidel’s test –ve. RE vision dropped to 3/60. He was managed by bleb revision and AC reformation with viscoelastic substance, steroids and cycloplegic drugs. During his follow up visit, BCVA on operated eye was 6/6 with IOP 14mmHg and is being maintained till 6 months after surgery

So, early and timely intervention is important to salvage an eye in post trabeculectomy complication
