FP1316 : When true and pseudo disc edema coexists : a case report

Abstract
Diagnosing pseudo disc edema does not rule out presence of underlying true disc edema. A middle-aged overweight lady presented with whooshing sound in both ears and unaffected visual acuity. On examination, both optic discs had blurred margins, suggestive of buried optic nerve head drusen. Neurologist confirmed normal MRI brain & CSF opening pressures and patient was given symptomatic treatment. On followup after 2 years, her symptoms recurred, this time associated with a drop vision on both eyes. Fundus revealed both eyes having frank disc edema. MRI brain was suggestive of Idiopathic Intracranial Hypertension along with raised CSF opening pressure. Treatment was started and on follow up, at 1 month, 3 months and then at year showed gradual resolution of disc edema. Thus, the diagnosis of a disc edema may often pose a diagnostic dilemma and sometimes may even coexist with a pseudo disc edema, leading to unfavourable circumstances if misdiagnosed.
