Abstract
We are presenting a case of unilateral impending CRVO in a 29 year old prisoner with no associated systemic risk factors. He had transient blurring of vision for 2 weeks in left eye with no associated pain, redness and photophobia. His pupillary reactions were normal with BCVA of 6/9 in left eye. Anterior segment was normal in both eyes. Left eye fundus exami-nation showed hyperaemic disc with tortuous vessels and few dot blot haemorrhages over posterior pole suggestive of impending CRVO. On OCT there was no macular oedema. Complete coagulation profile and thrombotic workup was normal, except raised homocysteine level (44.5 micro mol/L). He had megaloblastic anaemia with serum Vitamin B12 (12.54 pg/ml) and serum folate (1 ng/ml ) which were significantly low. He was diagnosed as Impending CRVO of young secondary to hyperhomocysteinemia because of malnutrition. Malnutrition in prisoners can be tip of iceberg, which can lead to disastrous complications.
