Abstract
Purpose: To evaluate morphological characteristics and intraoperative dynamics of posterior polar cataract (PPC) using intraoperative OCT (iOCT)
Methods: Prospective interventional study of 40 eyes with PPC. Primary outcome was morphology and intraoperative PC dynamics of PPC. Secondary outcome was PCR (compared with 72 cases of non-iOCT guided PPC)
Results: Three morphological variants of PPC – Type I (47.5%) with intact PC, clearance between PC and opacity, Type II (30%) with intact PC in periphery and shadowing in centre, Type III (22.5%) with shadowing and inability to delineate PC. Both hydrodelineation and dissection performed in type I PPC; only delineation in type II and III PPC. No PCR in type I PPC; 1case with type II and 2 cases of PCR with type III PPC. No significant difference in PCR between iOCT and non-iOCT guided surgery (p=0.7)
Conclusion: iOCT-guided surgery elucidates high-risk features in PPC, enables safe hydrodissection in type I PPC but does not prevent PCR
