Abstract
She had a history of migrane for which she was started on topiramate for last 1 month after which she developed the symptoms.
Her BCVA R/E was 2/60 and L/E was 2/60. On slit lamp examination R/E: Anterior chamber was shallow with circum-corneal congestion and swelling on eye lids. L/E: showed sluggishly reacting pupil. On refraction, myopic shift was noted. IOP measured through applanation tonometry was 28 and 26 mmHg in right eye and left eye respectively.
Ultrasound Bio-microscopy (UBM) was done in both eyes where ciliary body and iris tissue effusion was noted bilaterally. Topiramate was discontinued. Patient was started on steroid eye drops, atropine and anti-glaucoma medications and told to review after 1 week, where IOP had reduced to 14 and 10 mmHg in right eye and left eye respectively.
