Abstract
We describe clinical profile and surgical outcomes of patients with posterior lenticonus (PL) related cataracts in 85 eyes of 64 patients. 33% had bilateral PL. Median age was 3 yrs. 40% had total cataract. Sensitivity of immersion B scan to diagnose PL was 46%. Mean axial length was 21.5mm. Mean keratometry was 45mm. 64% patient underwent lens aspiration+ intraocular lens (IOL) in bag+ posterior capsulotomy+ anterior vitrectomy; 18% had IOL in sulcus. 58% treated for amblyopia. BCVA outcomes at 6 months were fair to poor in 2/3rd patients (median 20/100). 12% patients needed 2nd surgery (membranectomy, secondary IOL, pars-plana vitrectomy for cortex removal/ IOL repositioning).
We conclude, bilateral disease is more prevalent. Sensitivity of immersion B scan to diagnose PL is limited. Cataract extraction and optical correction with IOL is possible; with low intraoperative complications. The visual outcomes are fair to poor; likely secondary to late presentation and dense amblyopia.
