Abstract
In this prospective study of comparison between three intravitreal drug regimens, 64 patients of bacterial endophthalmitis were divided into 3 groups: group 1 received ceftazidime and vancomycin (29), group 2 received piperacillin-tazobactam and vancomycin (20) and group 3 received imipenem and vancomycin (15). After 48 hours of injection, patients were either observed or planned for vitrectomy. 62 patients underwent vitrectomy and only two were managed with intravitreal antibiotics alone. Based on improvement in vision at 3 months and final vision, all groups were statistically comparable. Among 15 gram-positive bacteria, only two Streptococci showed resistance to vancomycin, while among six gram-negative bacteria, two were resistant to ceftazidime.
Conclusion: Ceftazidime and vancomycin can continue to be the first-line regimen for bacterial endophthalmitis owing to their comparable antibiotic sensitivity and superior ocular bioavailability.
