Abstract

A 50 year old male presented to OPD with chief complaints of sudden onset blurring of vision in both eyes (BE) for last 15 days . The patient was a known case of AIDS and pulmonary tuberculosis for which patient he was on antiretroviral and antitubercular therapy. No history of ocular surgery present.On examination, best corrected visual acuity(BCVA) in RE was found to be 20/200, LE 20/200 . On anterior segment examination there was no abnormality, there were no cells in the anterior chamber or retrolental space. On fundus examination there were many cotton wool spots , pre-retinal haemorrhages, a macular star with blurring of disc margins in both eyes. On SS-OCT, macular edema noted in BE.

Clinical course: The blood pressure of the patient was checked and found to be 200/110 mm Hg. Thus a diagnosis of malignant hypertensison in a case of AIDS was made. Following blood pressure control the changes started resolving and the vision improved.
