Abstract
Acquired isolated sixth nerve palsy in children is caused by trauma, viral infections or tumors. The natural course of palsy can be studied by observing for long period of time. We report cases of sixth nerve palsy with different clinico-etiological presentation treated conservatively and kept on close follow up to study the chances of spontaneous recovery in children. Our first case was 6 year old child of bilateral sixth nerve palsy post pyogenic meningitis with horizontal diplopia and large angle esotropia with bilateral papilledema. He was treated conservatively with alternate occlusion for symptomatic relief. In 12-16 weeks abduction improved and angle of deviation reduced to 6 BO for which prisms were prescribed to treat residual diplopia which recovered fully . Similar spontaneous, progressive and significant recovery was also seen in a child of traumatic unilateral 6th nerve palsy proving the benign nature of the nerve leading to spontaneous recovery without recurrences.
