FP0461 : NYSTAGMUS LEADS TO DIAGNOSIS OF UNSUSPECTED PERIVENTRICULAR LEUKOMALACIA IN AN ADULT

Abstract
We report a case of periventricular leukomalacia who presented with latent nystagmus as the only clinical finding. A 19 year old male presented with complaint of difficulty in vocational binocular activities. On examination he was diagnosed with manifest latent nystagmus with adduction null and no other neurological deficit. MRI was suggestive of periventricular leukomalacia. Binocular vision was 6/6. Mild bilateral optic disc pallor was present. On sensory evaluation, fusion was present but stereopsis was less than 400 seconds of arc. Minimal manifest nystagmus in primary gaze, good binocular vision and absence of any other neurological deficit delayed the presentation of PVL and evaded the suspicion of presence of any neurological finding in this case. Though strabismus is commonly associated with manifest latent nystagmus, its absence should not lead to assumption of nystagmus being congenital. An MRI on slightest suspicion can lead to early diagnosis and management of PVL.
