Abstract

Introduction: Free cap is a rare intra-operative LASIK complication that is associated with flat corneas,inadequate suction, faulty microkeratome blades and surgeon related factors. There are two types of flap management, either conservative realignment with bandage contact lens (BCL) placement or flap suturing.

Aim: To study the risk factors and outcomes of two different management techniques of free cap.

Results: A total of 2129 microkeratome assisted LASIK procedure was done from January 2017 to March 2020 and there were 6 intra-operative free caps.Ablation was successfully done in all of them in the same sitting. Three of them were associated with flat cornea(K≤42) and one was done by a novice LASIK surgeon. Among the 6 free caps, 4 were sutured and 2 were managed by mere realignment and BCL placement.Analysis of post-op outcomes showed better and faster visual recovery with lesser need of post-op visits in those managed by mere realignment and BCL.
