Aim of Study :To note the corneal clarity and visual outcome after Phaco in fuchs with pre Op central corneal zero cell.Methods :This is a case of clear cornea phaco with monofocal foldable IOL of RE by Ozile dynamics under viscoat for NS3-4 in a 78yrs Indian Lady who is a Diagnosed case of OU Fuchs Enothelial Dystrophy with central corneal zero cell BCVA: RE 3/60,N36 and LE 6/9P,S/P PCIOL.RE Phaco was done in such a way thus minimal endothelial cells were damaged during surgery.Result : Immediate post OP was uneventful and 1month post OP clinically cornea was clear with BCVA 6/6, N6 and it is maintaining same over a year follow up.Conclusions: Phaco with central zero cell is always a surgically challenging scenario but we can achieve predictable out comes with clinically clear cornea easily by pre OP proper measure prior to surgery and minor modifications in surgical techniques -This is the surgical pearl of our presentation, leaving both Surgeon and patient contented
