Abstract
Purpose: To evaluate the outcome of Ahmed Glaucoma Valve (AGV) Decapsulation in patients with late surgical failure following AGV implantation
Methods: Formation of thick capsule around AGV footplate, if excessive can smother the filtration of Aqueous. Five AGV Patients with uncontrolled IOP despite 4 anti glaucoma drugs were included in the study. They underwent AGV decapsulation with excision of fibrotic tissue around the footplate. IOP and need for anti glaucoma drugs post decapsulation were recorded.
Results: Mean baseline IOP was 23.5mm Hg on 4 drugs. It reduced to 17.6mm Hg off anti glaucoma drugs, immediately, following decapsulation. At 3 months, 2 drugs were required in 4 of the 5 cases. There were no cases of Overfiltration or loss of Snellen acuity.
Conclusion: AGV decapsulation can help to reduce the need for drugs to attain the Target IOP and can be a cost-effective alternative to AGV replacement in management of uncontrolled IOP in cases of late failure of AGV.
