Abstract
Prospective interventional case series(16 eyes) of failed trabeculectomy to evaluate safety and IOP lowering effect of incisional goniotomy with MVR blade +/- phacoemulsification. relevant data analyzed for at least a 6-month follow-up. Success criteria- IOP ≤18mmHg with or without medications. 87.5% had severe glaucoma. Mean IOP change 24.37±5.77 mm Hg to 15.8±3.19 mmHg at 6 months (P=0.004); 37.14 ±14.87% reduction at last follow-up. Mean medications reduction- 2.81±0.91 to 1.27+0.9 (P<0.009). 2 patients had IOP <18mmHg with 0 medications (12.5%) 5 patients had IOP <18mmHg on 1 medication (31.25%) at last follow up. Complications-hyphema (25%) IOP spike (12.5%) cyclodialysis (6.5%). Cumulative survival probability (6 months)- 87.5%. if IOP< /=15mmHg with or without medication, survival- 68.75%. MVR blade assisted incisional goniotomy with or without phacoemulsification lead to significant reduction in IOP in failed trabeculectomy. It is a useful alternative to a retrabeculectomy
