Abstract
Primary congenital glaucoma is a surgical disease. Primary surgical options include goniotomy, external trabeculotomy and primary combined trabeculotomy and trabeculectomy (CTT). Recent advances in the surgical management of this disease include 360 degree suture trabeculotomy using 6-o’ prolene suture, ab-externo 360 degree trabeculotomy using fiber-optic microcatheter with illuminated tip, endoscopic goniotomy, and ab-interno suture trabeculotomy. In our country, CTT is considered the most suitable option given severity and advanced stage of phenotypic manifestation. In refractory congenital glaucoma, trabeculectomy with antifibrotic therapy such as MMC and 5FU is considered the most appropriate therapy by most surgeons. However glaucoma drainage devices such as AADI and Ahmed valve implant are emerging as suitable alternatives for refractory congenital glaucoma. Cycloablative procedures such as TSCPC remain the last resort. All these will be discussed with video demonstrations.
