Abstract
Aggressive management of complications by inexperienced cataract surgeons can lead to disastrous complications.Presenting a similar case where fishing of a sinking nucleus after posterior capsular rent & implanting intraocular lens(IOL) in an unstable bag led to disastrous outcomes.68 year old lady presented with retinal detachment after one week of cataract surgery done elsewhere.Giant retinal tear was visible in same clock hours as the main entry wound of phacoemulsification.No IOL was visualised on clinical or B-scan exam.On surgical table,posteriorly dislocated IOL became visible inferiorly,behind torn capsular bag in anterior vitreous.IOL explant with secondary IOL implantation in sulcus(power guided by IOL sticker from cataract surgery) was done to restore pseudophakia & to prevent vitreous substitutes from migrating anteriorly and hindering intraoperative visibility,followed by vitreoretinal surgery.Retina was attached with pseudophakia on 1st post operative day.
