Abstract
Breaking the monotony of routine cataract surgery, one fine day I stumbled upon a novel idea of using the anterior capsule lenticule, made after capsulorhexis, as a buffer to the sensitive endothelium to tide over the insult during phacoemulsification. This lenticule acting like a bioshield inspired me to apply this technique in cases of cataract with compromised corneas to enable endothelial protection, adding to the effect of viscoelastic. I was then motivated to use this procedure in a patient of Fuch’s endothelial dystrophy and cataract. She had a decompensated cornea in the contralateral eye following phacoemulsification and had undergone endothelial keratoplasty. The post-operative follow-up was very heartening which made my mind racing. I went ahead with more cases implementing this new shield, the anterior capsular lenticule. This simple technique kindled my interest, provoking me to share my learning experience of this bioshield with further modifications as situation demands
