Abstract
40 years old patient presented with healed keratouveitis with complicated cataract. There was central stromal corneal haze around 3.5 x 4.0 mm . Pupil was small and irregular due to posterior synechiae. IOP was normal. Phacoemusification with foldable hydrophobic IOL was planned. As pupil was not dilating with cycloplegic and mydriatic eye drop, iris hooks were used. Phacoemusification was done with stop and chop technique. Foldable IOL was implanted in the bag after thorough cortical aspiration. Iris hooks were removed and wounds were hydrated. Postoperative patient had 6/36 visual acuity. So complicated cataract can be managed even with small pupil by using iris hooks.
