Hypermature Morgagnian cataracts, though uncommon, post significant risks during surgery. They can have corneal oedema (in phacolytic glaucoma), calcification of capsule, turbid aqueous (leaked cortex), diffucult CCC due to presence of liquid cortex , no red glow, no cortex support to nuclues during emulsification along with weak zonules and brittle capsule . This video deals with different cases of morgagnian cataracts, presenting with a combination of multiple surgical challenges mentioned above. All cases are handled differently to get best results (including phaco, femto-phaco, Manual SICS and the role of vitreo retinal assistance).
