Abstract
Surgery of hypermature cataract is challenging and highly risky. Due to the innate nature of the anatomy of the lens-zonule complex, Morgagnian cataracts are at a high risk of complications at every stage of surgery. Posterior capsular rents due to weak PC and Zonular dialysis due to weak zonules are extremely common. Risk of nucleus drop into the vitreous cavity is also high. In this technique, the small nucleus is prolapsed into the anterior chamber after capsulorhexis and the IOL is placed in the bag, after which phacoemulsification is safely performed with adequate viscoelastic protection to the corneal endothelium.
