Abstract
A 42-year-old male patient present with complicated cataract in hie left eye with prior history of surgical PI done in the same eye when the patient was 10 years old. The patient had an iris tear measuring 2×3 mm in diameter with thinned out iris at 10-11 ‘o’ clock position, a poorly dilating pupil with posterior synechiae and a dense cataract with no view of the fundus. Left eye cataract surgery was planned under oral steroid cover and iris hooks were used during the surgery resulting in iris tear at the site of surgical PI due to iatrogenic trauma. Surgery was uneventful and foldable IOL was injected into the bag. By the end of the first week post-operative follow-up period, patient had an uncorrected vision of 6/9 and by the end of third week, anterior capsular opacification was noticed at the site of iris tear.
