FP0727 : Transient Attack With Permanent Consequences – A Perverse Harbinger of Future Morbidity

Abstract
A 60yr old lady with chief complaint of gradual, progressive loss of Vision in RE since 3 years. She is a hypertensive and diabetic for past 15 yrs on oral medication. Cataract surgery done in LE 12 years ago. BCVA in BE is HM. On examination RE had a normal Ant Segment, NS grade 4 cataract. Fundus no view. LE axial proptosis, prominent conjunctival vessels, RAPD, IOL in-situ. Fundus showed Non Glaucomatous Optic Disc Cupping. Macula appeared healthy apart from arteriolar attenuation. On further asking she reported there was sudden loss of vision in LE for past 4 months. FFA LE showed normal perfusion with arm retina time of 17 sec. SD-OCT showed inner retinal thinning with reduced foveal contour. Full field ERG in scotopic and photopic response showed b wave less than a wave. Diagnosis of Chronic Transient CRAO was made. She was immediately referred to a cardiologist and radiologist for further evaluation because of high risk of stroke in T-CRAO. Further ocular examination planned.
